It goes back to the Marvel Family in the Forties. After their success with Captain Marvel, Fawcett wanted more. So they started to de-unique the character, beginning with Mary Marvel, followed quickly by Captain Marvel Jr,* the three Lieutenant Marvels and so on. DC set about doing the same, with Superboy and later Supergirl, Kandor, the Phantom Zone, &c. The habit persists, tho few are aware of its origins anymore. _______ * Freddie Freeman had an odd problem. He gained his powers by saying “Captain Marvel”, not “Shazam”, which meant he couldn’t call Cap by name, or even introduce himself. It’s a problem DC didn’t seem to consider when they changed Captain Marvel’s name to Shazam.
