I know this is outside of the rules but....

John (and/or Sal) Buscema - JUSTICE OF AMERICA,  SUPERMAN (ACTION COMICS) 

Jerry Ordway/Roy Thomas - JSA/ALL-STAR SQUADRON set in the 1960s

A lot of times, creative teams were somewhat uneven.  We hardly ever saw the very best writer teamed up with the very best artist!

My all-time favorite writers never teamed up with my all-time favorite artists.  Any mixture of the following would have been welcome on any major title:

Neal Adams
John Byrne
Alex Ross
Jim Lee
Terry Dodson
Alan Davis

never teamed with--

Steve Gerber
Doug Moench
James Robinson
Alan Brennert
Steve Englehart (did he ever work on even one thing with Adams or Byrne--am I forgetting something?)

(I'm leaving out some other favorite artists like Paul Gulacy, Jim Starlin, Sal Buscema, Gene Colan, George Perez, and more because they DID team up with various great writers.)

Adams is said to have done some uncredited inking on Giant-Size Avengers #2, and the Crusty Bunkers inked some of Frank Brunner's pencils on Doctor Strange in Marvel Premiere #10 and 12–13.

James Robinson worked with Jim Lee on some WildCATS stuff.

I think WildCATS #19 with its main story by James Robinson and Jim Lee also had a backup story by Robinson and Terry Dodson.
