Fan Stuart Dietcher chronicled a visit to the home of Jack and Roz Kirby during the summer of 1993. That footage has been shared by The Jack Kirby Museum and Dietcher on YouTube:

No idea.

Incidentally, I don’t think it’s entirely accurate to say he was “aware” of OMAC. He expressed misgivings about one scene, and it seemed pretty obvious that he’d been fed that particular scene out of context.
