Who came up with the notion that, as a hybrid of Atlantean and Human, Namor's occasional rages were the result of an imbalance of staying too long on the surface?

I'm pretty sure it was Roger Stern, though JB was the first to execute the idea into the comics.

Yep, that was Rog (credited). It worked pretty well. Too long out of the water, oxygen overload. Too long under, oxygen deprivation.

I loved what you and Roger Stern did with Captain America! I wish you guys had been able to collaborate like that more.

Sounds like he would have made a good EIC!
The brilliance of that idea is that it addresses the villain-as-protagonist problem that has been discussed in other threads, brings a deeper  understanding of the character, and didn't require any pesky ret-conning that can be pretty triggering among fans.

When I started reading Marvel a big part of the appeal was that the characters were “down to earth”. Sure, super powers, but believable super powers. (Hey, I was 11!)

Well, at least they haven't made Spider-Man an armored wizard who can die and be resurrected, acting under the command of Sorcerer Supreme Victor Von Doom. That would be complicated!
