Interesting.
On a first glance, I like the rejected covers more than the actual ones except on iconic cases such as AMAZING FANTASY #15 and HULK SPECIAL #1.
But maybe that's conditioning? Generally, I'd go with the artist's first version as the correct one.

Can't say this was an exact replacement, but BLACK PANTHER got cancelled before issue #16 was published, but then the storyline was finished up in MARVEL PREMIERE, starting with an Al Milgrom/Klaus Janson cover.
The unused (Windeagle) cover is a pretty snazzy one by Rich Buckler & Bob McLeod.

Not loving that Buckler cover, but the Milgrom/Janson one is excellent.

Here's a nice Kirby cover for X-MEN #10...but there's something just a bit off with it. Ka-Zar looks good, but every X-Man is in an awkward pose, especially Cyclops.
Second attempt works much better! I assume Stan had Jack do a second try, and he was right!

Interesting! I’d say it was Ka-Zar who had the awkward pose on that unused cover. (One of the complaints I saw about ELSEWHEN was that I am “always” using the Savage Land. Given how far back the X-Men’s association with that chunk of real estate goes, I can’t honestly see my visits as terribly excessive!)

They were a lot of different stories that involved the Savage Land... not like they were the same stories. Anyway, Savage Land = visual excitement!
That #10 was the last one I managed to buy to have a run from #7-53, and it was a really sharp copy of it, pretty much the pride of the run. All sold sadly.
I loved that Black Panther cover by Milgrom/Janson (great coloring). I bought those three Panther issues as they came out, didn't know until now they would've been in his own title. I thought Jerry Bingham was great!

Well, yeah, the second Ka-Zar pose is better than the first one for sure, but it really looks like Cyclops is straining his neck to fire an optic blast behind him!
I really thought that Milgrom/Janson cover was Frank Miller until I looked it up.

Here's a fairly famous rejected cover by Neal Adams. He did not appreciate being wooed over to Marvel and then having his work tinkered with or criticized.
Here's the published cover.
The unused one was deemed confusing because the figures obscured the logo (on a book that wasn't selling anyway, so why not take a chance?). Somebody colored it in recent years. Looks a little less confusing in color, I think.

First Adams issue. I guess I can understand them not wanting the logo to be that obscured. Inspiration for the cover of #135 too right? They both really leap out at you, I'm sure the rejected one would've almost as much, also it shows Alex who is a major part of the story which the replacement doesn't. Not sure if any character outside The Metal Men were so much a part of a logo!
