By the way, I believe that Larkin just did a painted version of a John Romita pin-up from the Hulk’s magazine for the book cover.

I'm a big continuity guy, but, no, I don't need stories referencing the Hulk being grey or having three toes. As far as I'm concerned, he was green right from the beginning and had the full number of toes.
It's so much easier (and better!) to just say (if you even have to mention it at all) that the grey was a trick of the light that first dark night, and that the toes were also an illusion, with two toes so close together that they looked like one (twice, on each foot).
I long for the day when Peter David's entire run is retconned out of existence (like they did with the magazine stories, which were better).

I wouldn't go that far. I enjoyed PAD's Grey Hulk Saga. Ironically, it was far more "out in left field" than what JB was planning to do.
It was PAD's Merged Hulk Saga that was mostly boring. But by the end of his run we were back to the Banner/Hulk "relationship" as it was in the early 60s, more or less. With a green, smart (but not super-smart) Hulk.
Which, perhaps ironically, is where we are now in 2018 in THE IMMORTAL HULK.

Also, I really really liked how JB drew the Hulk during his all-too-brief run, even though it was supposed to be the "longer-haired Sal Buscema" Hulk. Still looked relatively Kirby-ish.

By the way, I believe that Larkin produced a painted cover based upon art by John Romita, so the Hulk’s particularly massive size on the book cover comes down to Larkin following Romita’s lead.

I remember being taken aback by John Byrnes Hulk entry for the OHOTMU. I was expecting the then current HULK by JB, but he displayed his Kirby homage version, which I wasnt quite prepared for.

I didn't read the Kirby version before years after I had read the Trimpe and Buscema versions. When I first started reading Hulk, which was then drawn by Trimpe, I was so young that when they switched to Buscema one or two years later, I actually can't remember if I noticed it or not. But I did notice when they started doing reprints of Trimpe after I had gotten used to Buscema. So it is the Buscema Hulk that is the defining version for me.
Also, the last issue of the original green Hulk was issue 271, with a short return in 272 (officially he has showed up again a few times, like in the crossroads storyline, but it doesn't quite feel like the old Hulk, and the comic had changed a lot at that point). And issue 292 was the final issue of the comic as I knew it. After that it went into a new direction that changed everything.
And who else felt that Rick and Betty betrayed Hulk? It was already established by Samson that Hulk was not just another version of Banner, even if Peter David would later change that, but an independent personality. Hulk was willing to do everything to help Rick and Betty when they were in trouble, even risking his own life. But as soon as he turned into Banner again, they told him they would do whatever it took to help him find a cure so he could get rid of Hulk permanently. At least it should have been some dilemma. (To Rick's defense, he tried to fix it by becoming a new Hulk, giving himself cancer instead.)

Sal's Hulk was my Hulk because I was reading the Defenders regularly, but not Incredible Hulk. So when I did pick up an issue, Herb's take looked off.
