Skimmed the thread to see what hadn't been guessed at yet. First the ones I had guesses for:
Joseph Gautier: IN THIS ISSUE: THE DEADLY DEBUT OF SILVER SHADE! ...AND FIRESTORM'S FIERCEST FIGHT EVER!
Don't remember any Silver Shades in Flash so probably a Firestorm. That's a Conway sounding villain and the one issue he did that isn't popping into my head is #53, so I'll go with that one.
Joe Hollon: DARK SWAMP, DEAP FEAR!
It's that Spider-Man/Man-Thing issue of Marvel Team-Up isn't it? #68? (You don't get recreations of the Marvel Tales version so I'll assume you got the real thing. :-) )

David Miller- "Rhino on the Rebound!" Marvel Romance #20. No, I'm just kidding... It's really The Thing #24. Or thereabouts.
Dave Phelps- Either Avengers 71 or Marvel Super Action #32. Based on the other comics you listed, I'd have to say MSA #32.
Joe Smith- Either Amazing Spider-Man #144 or Marvel Tales #121
A couple more that I believe are still unsolved...

Y'know, there's an unfortunate number of fans who came up through the Ultimate era at Marvel who are going to have to play this game in years to come by describing Ultimate Peter's action pose and which direction the light source is from...
"Okay, tough one, gang! Side bar is red, Peter's three-quarters of the way upside-down, right arm out, left leg bent behind his buttocks, light source: above right. Go!"

Brian Hague - weird, I remember copying over Blood Feud, but there it wasn't. Yours I just completely missed. :-)
Anyway, you're correct about mine (MSA #32) and I think yours was Showcase #94 (first appearance of the New Doom Patrol).

You are correct, Dave! I'm pretty sure there are a dozen or so comics out there somewhere with variations on "Final Battle" as cover copy, but that was the first one I could recall and the vintage seemed right. "Blood Feud" should be pretty common as well, but I can't specifically place one as yet.
The one with the flying capitol building I can almost picture, but maybe I'm just imagining it. :-)
