I also think this is an anomaly and not the norm. And I have to admit a bit of surprise for this too. Stan has ventured out from comics. maybe not that far. But he's been involved in television and movies, often in conjunction with superheroes. He has also started the Stan Lee Foundation in 2010 to focus on literacy, education and the arts. I could see this reaction maybe 25 years ago. But not today.
Oddly enough, at work comics is a big topic of discussion. A large majority of my work colleagues (like 25%) are actually well aware of the comics industry. I think this actually shows how much the industry has become popular even with those who have not read comics. I think this is due to the influx of superhero movies over the last 10 to 15 years.

As today involved me driving around and dropping Xmas presents to family, I thought I'd ask a few people the question (excluding my brother who likes superheroes).
Two of my three sisters didn't know who he is. However, my mum, who I doubt has ever read a superhero comic in here life* knew who he was. So I guess he does have name recognition. My stepdad knew who he was, too, and he was never a comic reader.
I think Stan Lee, Ronald McDonald, Hulk Hogan, Elvis Presley and a few other names are names that people JUST know even if they aren't interested in certain forms of entertainment. I am NOT for one minute comparing Stan Lee to Ronald McDonald, Hulk Hogan or Elvis Presley (all very different characters!) but say any of those names to people - and there's a chance they may know.
*My mum did read comics for girls like MISTY and BUNTY, but she assures me she has never read a superhero comic.

The only people I know that have heard of him saw MALLRATS in the 90's.

I'm willing to bet that even for the people who don't immediately recognize Stan Lee's name, many of them will know who you are talking about with some prompting.

I got a text message from a good friend today asking me the same question "Who is Stan Lee?"
I thought his fame reached beyond comic fandom!

Well, keep in mind there are plenty of athletes, actors, authors, singers, musicians, etc. that if you're somewhat informed in those areas you'll be aware of that many common civilians will have no knowledge of.
I'm kind of familiar with some popular musicians and singers but I would have a hard time picking Adele or Lady Gaga out of a lineup.

Yeah I think Stan isn't as famous as you thought. Certainly a giant in the industry but as an overall public celebrity - you have to DO something to get that famous. Star in a movie. Take a political stance. Help the needy. Lead Marvel Studios.
What does Stan actually do? What has he done for the last 30(?) years? Cameos and niche videos.

When I was in my early years at Marvel it was most frustrating when people asked what I did or had done and those blank stares would come into play when I said X-MEN or FANTASTIC FOUR or AVENGERS. Spider-Man they knew, as a character, as well as the Hulk. Both had been on TV. Now, I can say "X-Men" and people sort of know what I mean. Ditto for the rest. And THIS is why I am surprised that people don't know Stan. Easy name to remember. Repeatedly associated with these characters on entertainment show. Oh, well!

Out of the people who don't know the name Stan Lee. I wonder how many of them would recognize his face if they saw it.
