I don't mind children of friends calling me "Mr. Tom," though.
My daughters have a number of "honorary" uncles and aunts.

Yeah, but "Uncle Tom" has a pretty awful sound to it.

As far as the name convention goes...I agree with all that as been stated and add...
I work in a medical school and and always refer to the medical Doctors as Dr._____, not just as a sign of respect, but also that it helps the patients...puts their minds at ease etc.
I was raised in the south and we just always call people Sir and Ma'am as a matter of course. I also give up my seat on the train to older people and ladies.

I tell kids to call me "Brian" or Mr. Brian" ...either is fine. "Mr. Talley" is just too formal...and I'm just not built that way. I also throw around Sir and Ma'am regularly, and I still hold doors open for people, which seems to surprise some folks on occasion.

Children aren't allowed to speak to me, so it's already solved the whole "Mr.
Cook"/"Sir"/"Al" thing.

My wife once advised her students that if they did not make eye contact with adults, they would become almost invisible...they tried it and found it to be true.

Its not the holding open that surprises them, its the way you close the door as they are walking through it . . . .
