The mouth is open and those might be Mac's teeth that you can see at the bottom.

What's funny is, after having that comic for 24 years, I only noticed this tonight; and I almost jumped out of my seat when I spotted it.
I don't think this is a case of "cloud-watching".

I think it's more like this...

it looks like Mac is turning his head towards the right...towards Heather.
(okay...maybe not as happy.)

I'll admit the bottom part could be a neck, shoulders, and jawline but the upper part loses me. It doesn't quite have the grotesque effect going for it either.

Holy crap! I think Brad's version finally did it for me!!!! A little confirmation from JB would finally put this old ghost to rest for me. We'll see.
If Brad and Darren are correct, it is a rather gruesome image.

I think Greg's version is correct - it is reminiscent of a similar panel in Fantastic Four when Doom's assistant is destroyed by the Power Cosmic.

I think he's face-forward, mouth open in agony, but I see what you mean with the jaw and his head turned, but I still think he is face-forward.
