The Leader kills alot and no one cares. Why the fuss over the Hulk? It's the good old: "Let's turn the good guys into bad guys 4 no clear reason" stock cliche #2.

Of course it's ridiculous. But The Hulk is a pretty ridiculous character. Gamma rays give you cancer and death, and he is a walking contradiction of several laws of physics.
So, where do you draw the line? I don't buy the whole respect the "conceit of the genre" concept. That only leads to dull comics. Challenge it. Break it. Tear the whole conceits apart. The best work is always because of that. But give me something clever in return. Something brilliant. THINK ABOUT WHAT YOU'RE DOING.
And I don't see that in questioning the nature of Hulk's rampages. If you turn him a murderer, the whole character falls apart. Why should we emphasize with the plight of an unthinking, murder machine? Why has he been allowed to be free as long as he has? That makes the entire superhero community in the MU useless. And it doesn't serve any purpose. Doesn't make his stories more interesting; nor does it could drive future stories better than the not killing Hulk.
There are two fundamentally different approaches to "Realism" in comics. The one from Watchmen, or The Authority or The Ultimates. Where superheroes are treated as how they would be in our world. But with the exception of Watchmen, I never really seen it to work particularly well.
The other is what I call The Astro City approach. Real people living in their world. That's the POV I prefer these stories to take. In the first one, yeah, of course the Hulk would kill thousands of people. In the second, the people living in their world know better to evacuate when a big fight is happening at their doorsteps.
I don't mind realism in comics. As long as it serves a purpose. I can't think of any a killer Hulk could have.

I am not sure what the bold part means. They are treated as the writers think, not like they would in the real world. There is simply no fucking way to know this.
Hulk doesn't kill, at least knowingly (which is included in the original post, I think, and which excludes the Hulk-separated-from-Banner version that Mantlo and Byrne explored).
If I were writing the stories, he would never, ever kill. That's the Hulk I refer to read about.

And that's the key to me: The Hulk *is* a hero ultimately. He's an angry, mad brute but deep down, his intentions are good. He's loathed by the masses based solely on the first impression of him (hulking monster) -- even Banner, ironically enough, is guilty of this.

If Hulk would existed in the real world, he surely would kill people. A big, ravaging monster who wants only to be alone would kill everyone in his way.
In the real world.
But in COMIC BOOKS... You know, those funny pages not related to the real world, Hulk never killed. I don't care for what they tell at Marvel.
