I have not read much of Sal's Cap, but Sal is the Hulk artist in my book.  So my vote:

Did the Hulk ever have a nickname for Captain America?

I know "Winghead" has been used, but I didn't know if it was the Hulk who used it. Shield-Man sounds like something the Hulk might say, but I don't recall it being used.
