There was an issue of Daredevil, #140 (I think), where he has to fight both the Beetle and the Gladiator. DD takes some lumps and comes close to losing. A real brutal fight as I recall, especially for that day and age.

Gotta go with Dave on the Batman and the Outsiders #19.  That was nicely done. If you haven't read it, you should. 

I liked the way in which the Superman/Hulk battle went in Superman vs. Spider-Man #2 with the possibility that things could've quickly gone much, much worse for Superman in very short order.

JB's Thing vs. Thing in Marvel Two-In-One #50 was fun. "Bah.  Will I never know peace?"

Beta Ray Bill vs. Thor in #337 & #338.

There's a Don Newton illustrated Shazam story in World's Finest wherein we're invited to guess the identity of the foe and the answer makes the fight a satisfying one.  If ever anyone had a punch in the mouth coming...

The Thing and tthe
Hulk were always incredible for me, Supes and Ali was beautifully, yet agonizingly rendered.... my favorite is DD vs. the Hulk... brutal!

Incredible Hulk #300 by Mantlo/Mignola was one hell of a slugfest between the Hulk and the Avengers...and just about everyone else.

Gladiator vs the FF had some of the best visuals ever.

Silver Surfer vs Thanos in SS #50 was unique.

Odin, Thor, and Loki vs Surtur was epic.

In some ways, when you strip comic books down to their core essentials, their true joy comes down to eternally debating the "Hulk vs. Thing" arugment. My own favorite H/T battle came in Giant-Size FF #1 when they switch minds--awesome! And it's really disappointing that JB never gave us a Hulk/Thing battle; it would almost certainly have been a definitive one.

You ever notice most of the really cool slugfests involve the Hulk:

Hulk vs Silver Surfer: Tales to Astonish 93, Hulk 250

Hulk vs Namor: Tales to Astonish 100, Defenders 52

Hulk vs Thor: Defenders 10

Hulk vs Thing: Fantastic Four 25, Marvel Feature 11 

Of course there were some other notable slugfests:

Silver Surfer vs Thor: Silver Surfer 4, Thor 193

Thanos vs Everyone: Captain Marvel 32, Marvel Two in One Annual 2

and my all time favorite humorous slugfest:

Hercules vs Thor: Thor 356
